NOTE 1: The "Medical Condition" field is optional. If chosen, it will use 2 lines of your tag's available line limit (limits are listed next to the tag shapes). A medical icon will display next to the information you provide. There is a 12 character limit for this option.

NOTE 2: If you choose to request a proof of your tag, the proof will be sent to your registered email address in PDF format. If no reply is received in 5 business days, the order will be processed as is.
